2011 Empire Passport Application
Your key to all-season enjoyment at New York State parks is the Empire Passport, providing unlimited day use vehicle entry to most of New York's State parks and recreational facilities. The 2011 Empire Passport is accepted now through March 31, 2012.

Season Pass Application

Group Rental Form Waiver
Guests age 17 or younger who will be visiting Belleayre Mountain with a group and without a parent or legal guardian must bring this signed form with them on the day of their visit in order to rent equipment.

CSEA Membership (PDF) - Only to be filled out to join the CSEA Union. This union represents all Belleayre Mountain Empolyees except for certified Snowsports Instructors and Ski Patrol members.
PEF Membership (PDF) - Only to be filled out to join the PEF Union. This union represents only Belleayre Mountain PSIA/AASI certified Snowsport Intructors.
