Federal and State Forms

  • Form W-4 - Federal Employee Withholding Allowance Certificate.  Complete so that your employer can withhold the correct federal income tax from your pay.
  • IT2104 - New York State Employee Withholding Allowance Certificate.   Complete so that your employer can withhold the correct state income tax from your pay.
  • IT2104-E - Certificate of Tax Exemption from State Withholding.  Submission of an accompanying W-4 form reflecting federal exemption status is required.
  • USCIS Form I-9 - Employment Eligibility Verification Form.
